I think this is one of the best. New England/East Coast oriented, but has a very good discussion of the life histories of caddis, mayfly, and stonefly bugs.
After the life history section at the start of each chapter, the left page shows a picture of the bug (all taken by Ames on the White River in Vermont) and the right page shows flies that match it, along with tips for fishing them.

I like the books by Dave Hughes and Rick Hafele, such as "Handbook of Hatches" and especially "Western Hatches" to help put what I observe to fishing use.
