My catches were all on the same day:

Well-known Western creek in Montana: brown, brookie, cutthroat, rainbow, whitefish and a bull...

Local Western WA. lake: perch, bass, crappie, rainbow and a catfish ---> on the same fly...

Edit: I forgot the whitefish.

Up north many years ago on Lake Roesigor, this lake is like three separate lakes, I caught RB trout, bluegill, perch, largemouth bass all on a gold cone headed olive type leech pattern. Finished the day off catching a few Kokanee but that was with conventional gear and the wedding ring spinner set up. Not really any grand slam of significance but a fun day nevertheless.
 
The “Grand Orifice” gave up a Rainbow, Brown, Cuttthroat and Grayling in one float. Was missing the brookie but 4/5 is close. Maybe this year.

Edit: forgot the mighty whitefish!

Redband, Whitefish, Steelhead, Bulltrout, all on the same bead head hares ear.
Stocker trout, crappie, bluegill, redear w/ largemouth for +1 and brown bullhead for +2. All on the same black woolly bugger!

Rainbow, Brown, Brook and an Atlantic Salmon one summer evening on East Lake in the mid 2000's. Just three of us float tubing the SE corner when a Black Caddis hatch went off and the fish converged, smashing everything in sight. I'd previously hit the trifecta, first time I was able to add an Atlantic in the mix as well.
 
Last October with @Northern on a Gifford Pinchot lake I caught brook, brown, cutthroat and tiger trout in the same lake on the same day on the same fly (a size 14 Adirondack Muddler). That was a treat, especially since I'd never caught a tiger trout before!

For those who question this I have been there to see it done. Forgot camp made Thompson Canyon tomato salsa, however, and perhaps some jalapeño poppers. And beer should be plural.
 
A friend's ice-fishing slam: 40+ Perch, 2 Walleye and an outdrive complete with stainless steel prop.
